This summer is the first season in which a significant proportion of our patients at Thames Dental & Skin are managing their face and skin through considerable physiological change. GLP-1 medications, intermittent fasting, bariatric surgery — the route varies, but the result is the same: rapid weight loss that reshapes the face faster than the skin can adapt.
The press calls it “Ozempic face.” I prefer rapid-weight-loss skin — because the changes are not about a drug, they are about biology. Fat pads that quietly support the face begin to diminish. Skin loses tautness. Cheeks hollow, jawlines soften, tear troughs deepen. Laxity appears in the upper arms, abdomen and inner thighs. None of this resolves on its own.
There are real benefits to acknowledge. GLP-1 medications can improve insulin sensitivity, reduce systemic inflammation and slow glycation — all of which support skin quality at a biological level. The metabolic foundation often improves. The structure of the face does not.
This is where I see the most common mistake. Patients return to a clinic asking for “more filler” or “the latest skin booster,” and what they actually need is a phased, sequenced plan — one that rebuilds structure first, restores skin quality second, and is anchored from the inside out.

The four pillars of the Thames Facial Reset
1. RESTORE STRUCTURE — Dermal Filler
The face after rapid weight loss has lost something architectural. Bone-adjacent fat compartments that quietly support the cheek, temple and jaw have shrunk. Volume needs restoring where the foundation has given up – not merely where the skin has hollowed.

3. REGENERATE — Polynucleotides
Polynucleotides are my treatment of choice for skin that has thinned rather than simply aged. They work at a cellular level — repairing fibroblast function, enhancing hydration, and stimulating fresh collagen production without adding any volume.
The under-eye area, the neck, the back of the hands — anywhere the skin has lost density — these are where polynucleotides quietly do extraordinary work. Particularly valuable for patients whose faces have become noticeably more delicate after weight loss.
4. THE FOUNDATION — The 360° Assessment
No facial treatment plan should begin until the foundation is understood. Patients who have used GLP-1 medication need an in-depth clinical review of metabolic markers, nutritional status, hormone balance and skin condition before a treatment plan is agreed.
Without that baseline, we are working in the dark. This is the part most clinics skip. It is also the part that makes the difference.
Sequence and timing matter
The most consistent pattern I observe is this: volume loss and skin laxity do not resolve on their own, and the patients who do best are those who begin treatment early — not a year later, wondering what went wrong.
A phased Thames Facial Reset typically begins with structure (fillers), is followed by regeneration (polynucleotides) and tightening (RF microneedling), and is supported throughout by metabolic and nutritional review. We schedule formal reviews at three, six and twelve months, because skin remodelling and tissue regeneration are cumulative — the trajectory matters more than the single result.
